JOB RESPONSIBILITIES

 Monitors contractual compliance, provides administrative oversight and clinical consultation to assigned targeted case managers. Reports to TCM Program Director.

 PROCESSING DUTIES

  • Reviews notes and documentation submitted by targeted case managers.
  • Performs random charts audits as part of departmental QA process.
  • Coordinates school supplies and donations and maintains accurate records.
  • Monitors transition planning and discharges to ensure contractual compliance.
  • Completes LAURIS deactivation form after ensuring chart is CARF and Medicaid/HMO compliant.
  • Conducts internal satisfaction surveys by calling families served by the TCM program, and maintains records.
  • Ensures TCMs follow incident reporting procedures compliant with Federal and State Law and submits incident reports to the QA Department.
  • Provides customer service to providers and clients by making appropriate interventions and documenting such interventions.
  • Provides consultation to direct providers to ensure proper client service provision.
  • Follows HIPAA and confidentiality rules.
  • Follows MPCI and CARF policies and procedures.
  • Provides direct care services to clients as assigned.
  • Attends Staff and management meetings as required by Agency.

 OTHER DEPARTMENT DUTIES AS ASSIGNED

  • Attends and participates in staff development trainings and workshops.
  • Enter data as required in the Lauris database system.
  • Attends meetings and trainings in the community.
  • Generate correspondence as required.

EDUCATION REQUIREMENTS

  • Bachelors Degree in Social Work or related field with 3 years of experience in Medicaid and Child Welfare Services. Master’s Degree preferred. Bilingual (Spanish and/or Creole) preferred.
  • CBHCMS Certification Mandatory.
